Who is Inquiry Counseling?

Hello! I'm Jack, a queer, trans, and neurodivergent therapist based in Salt Lake City, UT.  For over 20 years I've worked in various roles within the queer community. In 2014, I graduated from the University of Utah with my MSW and have primarily worked in individual counseling settings with LGBTQIA+  teens and adults. I've had the privilege of companioning clients in their gender transition goals including TGD youth as a Certified Transgender Treatment Provider in the State of Utah in compliance with Utah State Code 58-1-603.

  • What is your licensure?
    I graduated with my MSW from the University of Utah in 2014 and have been licensed as an LCSW in the state of Utah since 2016. I am also a Transgender Treatment Certified Mental Health Provider in the state of Utah in compliance with Utah Code 58-1-603. This allows me to provide letters of support for clients under the age of 18 seeking gender-affirming medical care who have been diagnosed with gender dysphoria prior to January 2023.
  • What are your specialties?
    I specialize in working with trans and gender diverse individuals across the lifespan. As a transgender person myself, I take this work to heart and consider it a privilege to companion others exploring their gender and pursuing what transition looks like for them.
  • Where are you during our sessions?
    I provide telehealth from my home office. The environment is equivalent to the same sort of privacy and confidentiality an in-person office would provide.
  • What ages do you work with?
    I work with clients ages 13+. For minor clients, I ask that parents join the intake session and commit to regular check-ins to review progress. I am a Certified Transgender Treatment Provider in the state of Utah in compliance with Utah Code 58-1-603, allowing me to provide letters of support for youth with an established gender dysphoria diagnosis prior to January 2023.

    You are welcome to email or call me to discuss what services you're looking for and scheduling.
  • What kind of therapy do you do?
    My therapeutic approach is multifaceted & trauma-informed. The bulk of my training and practice has been centered in Feminist Multicultural Therapy (FMT), Acceptance & Commitment Therapy (ACT), Mindful Self-Compassion (MSC), as well as a sprinkling of narrative, existential, & somatic approaches. More important than the approach, though, is the therapeutic relationship. Therapy is collaborative and your feedback on what works and doesn't work in sessions is just as important as my education and experience.
  • What are your fees?
    I currently charge $200 for the initial intake session and $170 for subsequent sessions. Sliding scale options are available on a limited basis. Please don't hesitate to ask me about discounted rates should finances be a limitation. Though I cannot guarantee availability, I understand that therapy is expensive and want to find ways to accommodate you if at all possible.
  • What insurances are you paneled with?
    I am currently in the process of updating my credentialing status with several insurers.
  • How do you manage billing?
    I am partnering with a third-party biller to manage claims and all other billing concerns.
  • Questions to ask your insurance provider
    Do I have out-of-network mental health benefits? What amount will I be reimbursed for the following services with a Licensed Clinical Social Worker ? CPT (Current Procedural Terminology, or Procedure): CPT Code 90791 (initial consultation) at a fee of $200.00 CPT Code 90837 (subsequent psychotherapy sessions) at a fee of $170.00 per session Is there a deductible I need to meet each year before I can begin to receive reimbursement for sessions? Is there a session limit per year? Is a pre-authorization required, and if so what information do you need for this? Is teletherapy covered by my plan? What (if any) diagnostic codes are NOT covered? Are progress notes/medical records necessary for reimbursement?
  • What happens in an intake?
    We'll meet for 60 minutes to talk about what brings you to Inquiry Counseling, your story, and what you hope to get out of therapy. I'll answer any questions you may have about what to expect out of therapy and hopefully have a chance to introduce you to my cat, Ginsberg, in the process.
  • How long do sessions last?
    Individual sessions are 55-minutes.
  • How many times will we meet?
    We will collaborate on a session frequency that meets your needs. This may fluctuate over time, but my endeavor is to provide you with the care you need at the pace that feels right for you.
  • What is your cancellation policy?
    I ask that at least 24 hours advance notice be given if a session needs to be canceled. If a session is cancelled late, there is a $25 fee; if a client no-shows an appointment, a $50 fee will be charged. *Please note that late-cancellation and no-show fees are not covered by insurance*
  • Where should I be during our sessions?
    You will need to be in the state of Utah to receive teletherapy. Please ensure the location you are in is private with minimal distractions and not in a moving vehicle.
  • What are the limitations of confidentiality?
    Anything said in therapy, stays in therapy. However, there are some exceptions. Should I be concerned about your or another's immediate physical safety, I will first discuss with you ways to ensure safety. If imminent risk remains present, then I will get in touch with your emergency contact and/or crisis supports to assist you. Similarly, if we discuss abuse of a child or vulnerable adult, I am mandated to make a report to Child or Adult Protective Services. Lastly, if a court order or subpoena is issued, I am required to release records. These are covered in more detail in the informed consent document in your intake documentation. Prior to my taking any action, I will discuss the steps I will take with you. Any disclosure of records outside of these exceptions will require a signed release of information from you.
